How website information is handled

This page explains the known data flows used by this replacement website. It is not the final privacy notice and requires formal review before launch.

Who is responsible

The website represents 4 Great Stuart Street Dental Practice. The exact legal entity and data-controller wording have not yet been confirmed and must not be inferred from this page. Privacy questions can currently be sent to info@4greatstuartstreet.com or by post to 4 Great Stuart Street, Edinburgh EH3 6AW.

Information you choose to provide

The website embeds the practice’s existing forms for appointment requests, new-patient registration, general contact, urgent dental queries, medical history and feedback. Those forms may collect contact, identity, appointment and health information depending on their purpose. Only provide information relevant to the selected form.

Form and content providers

The embedded online forms are hosted and processed by Zoho Forms, including the practice-branded forms.edinburghdental.net address. Zoho receives technical connection information when you load a page containing a form, and form submissions pass to that provider before reaching the practice. An external-tab fallback is also available. The patient-article page loads a script from SORO automatically. When you visit the article library, SORO receives technical request information such as your IP address, browser details and the referring page, and supplies the current article list, article bodies and any featured images. The practice must complete its provider and privacy review before launch.

Website analytics and cookies

This replacement does not include the previous site’s custom session-replay analytics or AI chat assistant. It does not add advertising pixels or optional analytics by default. The hosting platform may process essential request logs, and Zoho may use necessary storage when an embedded form loads. The practice should review those providers before launch and add consent controls before introducing any non-essential tracking.

Why information is used and how long it is kept

Information submitted through a form is intended to let the practice respond, arrange care, register patients, prepare for safe treatment, handle urgent enquiries or review feedback. Clinical and administrative records are subject to the practice’s retention obligations. This website does not invent a retention period; contact the practice for the current record-retention policy.

Your choices and rights

You can contact the practice to ask about personal information it holds and to request access or correction. Other rights can depend on the reason information is held and legal or clinical record-keeping duties. If you are dissatisfied with how a data-protection concern is handled, the Information Commissioner’s Office explains how to raise a complaint.

Do not use a web form in an emergency

Online forms are not monitored as emergency services. Call 999 for a life-threatening medical emergency. For an urgent dental problem, call the practice during opening hours or follow current NHS Scotland dental-emergency guidance.

Launch review required: the business/data-protection owner must confirm controller identity, provider contracts, legal bases, retention, rights handling and any additional clinical privacy information before the production domain is connected.
